Gifted ADHD in British Columbia: When a Bright Child Is Still Struggling

“I know my child is bright. Why is so much still so difficult?”

At Breakthrough ADHD Center, I work with many bright children. Some have been formally identified as gifted or twice-exceptional, often called 2e. Others have not been assessed, but their intellectual strengths are clear to teachers, family members, and peers.

Their parents describe a child who learns quickly, asks unusually thoughtful questions, or becomes deeply absorbed in a subject that matters. The same child may still struggle to begin schoolwork, organize an ordinary routine, recover from frustration, or show what is understood on the page.

For over 20 years, I have worked with neurofeedback and brain training. One distinction has remained especially important in my work with gifted and bright children: intellectual capacity and reliable access to that capacity are not the same thing. A child can understand far beyond grade level and still need substantial support to activate attention, hold a sequence in mind, regulate emotion, and produce work consistently.

Why can a bright child still struggle with ordinary work?

A parent may describe the problem quietly: “The understanding is there. Getting started and getting it onto the page are the difficult parts.” The child can explain the concept aloud, yet the document remains blank or the first question is still unfinished after repeated returns to the task.

Parents in our clinic often report that starting an assignment can take one or two hours every school day. That is a clinic observation, not a population statistic. Its importance is the frequency: the family is not managing one difficult afternoon, but a recurring gap between what the child knows and what the child can independently initiate.

Working memory can be one part of that gap. It allows a child to keep relevant information active while reorganizing it, updating it, or using it to guide the next step. In a study of children ages 8 to 13, the ADHD group showed substantial difficulty in some working-memory components, but the profiles also varied considerably from child to child (Fosco et al., 2020).

This variability matters. A strong vocabulary, advanced reasoning, or exceptional knowledge in one area does not tell me how easily the same child can shift into a less interesting task, keep the goal active, sequence the work, and continue when the first effort feels uncomfortable. It also means that one unfinished assignment cannot identify the cause.

Why can giftedness make ADHD harder to recognize?

Giftedness can make the picture look better from the outside. Strong reasoning may help a child compensate, answer correctly, or achieve an acceptable final result. The result can hide the time, prompting, redirection, and emotional effort required to produce it.

Cadenas and colleagues studied children and adolescents with ADHD and high intellectual ability in two independent samples. They found that ADHD-related cognitive differences were broadly similar in type and severity to those seen in average-intelligence participants with ADHD, while some differences were less apparent when highly intelligent participants were compared with average-intelligence peers rather than peers of similarly high ability (Cadenas et al., 2020).

That finding helps explain a pattern I watch closely. A child may appear average or even successful in a classroom while functioning well below what the child’s reasoning ability would lead others to expect. The relevant question is not simply whether the work was completed. It is how independently, consistently, and sustainably the child could access the ability needed to complete it.

When performance is excellent in one moment and unexpectedly weak in another, adults may conclude that the child could do it if sufficiently motivated. Sometimes motivation, interest, confidence, or the fit between the child and the task does matter. But inconsistency should not automatically be reduced to poor willpower or defiance.

Why can home and school show different children?

This concern often comes up in a school conversation. A parent may say, “The teacher feels the work is generally fine. I would like to understand what is happening before it reaches that point.” The parent sees the long delay before school, the work that stretches into the evening, or the emotional release after the child has held everything together during the day.

Different observations don’t necessarily mean one adult is mistaken. Home and school place different demands on attention, independence, transitions, and emotional control. They also provide different levels of structure and different opportunities to observe the child before and after the visible result.

A study of 92 children ages 6 to 16 compared ratings from parents and teachers of gifted children with ADHD, gifted children without ADHD, and non-gifted children with ADHD. Parents reported more inattention in their gifted children with ADHD than teachers did, supporting the value of combining information from multiple settings (François-Sévigny et al., 2022).

This is why I take the parent’s account seriously without turning it into a diagnosis. The completed page, the classroom behaviour, the home routine, the child’s strengths, and the support required are all parts of the same picture. No single observer sees the whole pattern.

Parents often carry this concern with considerable restraint. In a small 2026 qualitative study of 11 parents of intellectually gifted children with ADHD in Quebec, parents described constant supervision, uncertainty, advocacy, and concern that discussing giftedness could be perceived as pretentious (François-Sévigny et al., 2026). The study was small, but the themes help explain why some parents minimize the strain even as they continue to search carefully for answers.

What does the hardware side add to the explanation?

I use a simplified hardware-and-software analogy to make a complex picture easier to discuss. It is an analogy, not a literal division of the brain and not a diagnostic model.

The software side includes learned skills, communication, motivation, confidence, relationships, interests, curiosity, and the strategies a child has developed. Development, sleep, nutrition, physiology, sensory load, and the environment can also influence how the child functions. These elements matter, and they often interact.

The hardware side refers to the neurophysiological activity that supports shifting, sustaining, and adapting attention and regulating arousal and mood. Brainwaves are one way of observing part of that electrical activity. They are not the complete explanation, but this category is often missing when a family has been told only to increase effort, consequences, or motivation.

The analogy changes the question. Instead of assuming, “Why will this child not do it?” I ask, “What may be making reliable access to attention and regulation difficult in this situation?” That question leaves room for brain activity, cognitive functions, emotion, learning, relationships, health, and context without declaring that one factor explains everything.

When considering neurofeedback training, I recommend assessment first. Neurofeedback does not diagnose giftedness, ADHD, or ASD. Assessment helps determine whether the child appears to be an appropriate candidate and what type of training plan may be relevant; formal diagnosis must come from an appropriately qualified professional using clinical information from more than one setting (Wolraich et al., 2019).

In our model, parent coaching is as critical as the neurofeedback training. Parents need help connecting training observations with patterns at home and school, understanding possible contributing factors, and responding in ways that do not treat every difficulty as willful behaviour. Training the child without strengthening the parents’ understanding would leave an important part of the work unfinished.

What should the first conversation clarify?

A useful first conversation does not begin by assuming that every bright child has ADHD or that every inconsistency requires neurofeedback. It begins with the pattern: where the child functions strongly, where access to that ability becomes unreliable, how often the difficulty occurs, what support is already required, and how home and school observations compare.

For some families, the next step may be an assessment of candidacy and functioning. For others, a formal psychological or medical evaluation, counselling, parent support, educational planning, or another form of care may be more appropriate. The purpose is to organize the questions before deciding what the child needs.

Frequently Asked Questions

What is gifted ADHD?

Gifted ADHD is an informal phrase for a child who has significant intellectual strengths and ADHD. When giftedness and a disability or neurodevelopmental condition coexist, the child may be described as twice-exceptional or 2e. Gifted identification and ADHD diagnosis are separate processes, and one should not be assumed from the other.

Can a child be bright without formal gifted identification?

Yes. Some children have clear intellectual strengths noticed by teachers, family members, and peers but have not completed a formal gifted assessment. Those observations are useful, but they do not confirm giftedness or ADHD. The child’s strengths and difficulties can still be discussed while the family considers whether formal evaluation is needed.

Why can a bright child struggle with ordinary schoolwork?

Advanced reasoning does not guarantee equally advanced task initiation, working memory, organization, attention regulation, or emotional regulation. A child may understand the material and still struggle to access that knowledge consistently under everyday demands. The cause must be explored rather than inferred from the unfinished work alone.

Why can parent and teacher observations differ?

Home and school place different demands on the child and provide different levels of structure. Parents may see the preparation, recovery, and repeated support surrounding a task, while teachers may see the classroom result. Both perspectives can be accurate, and combining them gives a fuller picture.

Can neurofeedback diagnose giftedness, ADHD, or ASD?

No. Neurofeedback is training, not a diagnostic procedure. EEG and training information may contribute observations about brain activity, but a single pattern cannot confirm giftedness, ADHD, or ASD. Formal diagnosis or gifted identification requires an appropriately qualified professional and a broader evaluation.

Are services available outside Vancouver and Langley?

Breakthrough ADHD Center has locations in Vancouver and Langley and may work remotely with appropriate families elsewhere in British Columbia, across Canada, and internationally. Remote suitability depends on the child, family circumstances, local requirements, and the service being considered. An initial conversation and assessment help determine whether a remote training plan is appropriate.
